I give my permission to receive massage therapy.

I understand that the services offered today are not a substitute for medical care.

I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.

I affirm that I am in suitable health to receive massage and will inform my therapist of any conditions that could affect treatment.

I understand that the information collected on my intake forms is for practitioner use to ensure my own safety.

I affirm that I have notified my therapist of all known medical conditions and injuries.

I understand that by choosing session enhancements such as herbal topicals or aromatherapy that I am responsible for knowing any personal sensitivities or contraindications.

I understand the importance of informing my massage therapist of all medical conditions and medications I am taking that may contraindicate massage/bodywork and to let the massage therapist know about any changes to these.

I agree to inform the therapist of any changes in my health and medical conditions. I understand that there shall be no liability on the therapist’s part should I forget to do so.

I understand the risks associated with massage therapy include, but are not limited to: superficial bruising, short-term muscle soreness and/or exacerbation of known and/or unknown injury.

If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort.

By signing this release, I hereby waive and release my therapist from any and all liability, past, present, and future relating to massage therapy and bodywork.

I understand that massage is entirely therapeutic and non-sexual in nature.

I understand that any illicit or sexually suggestive, flirtatious remarks, advances, hints, or jokes made by myself (client) at ANY juncture will result in immediate termination of the session and/or client/therapist professional relationship, and I will be liable for payment for the full scheduled appointment.
