I give my permission to receive massage therapy.

I understand that therapeutic massage is not a substitute for traditional medical treatment or medications.

I understand that a massage therapist does not diagnose illnesses or injuries, or prescribe medications.

I have clearance from my physician to receive massage therapy.

I understand that the information collected on my intake forms is for practitioner use to ensure my own safety.

I understand the risks associated with massage therapy include, but are not limited to: Superficial bruising, Short-term muscle soreness and/or exacerbation of undiscovered injury.

I understand the importance of informing my massage therapist of all medical conditions and medications I am taking, and to let the massage therapist know about any changes to these. I understand that there may be additional risks based on my physical condition.

I understand that it is my responsibility to inform my massage therapist of any discomfort I may feel during the massage session so he/she may adjust accordingly.

I understand the importance of informing my massage therapist of any allergies and/or sensitivities to any topical products, foods, herbs, plants, chemicals, etc.

I am aware that herbs can have many contraindications and that it is my responsibility to understand what herbs may be contraindicated during any session

I understand that there are contraindications for infrared saunas common but not limited to: Hemophilia, Insensitivity to heat, Pharmaceutical drugs, Alcohol, Cardiovascular conditions, Elderly, Fever, Implant, Joint injury, Outfitted with pacemaker or defibrillator, Children, Menstruation, Under 12 years old, Chronic conditions, Diabetes, Disease, Brain tumors, Epilepsy, Skin infections

I release the company and the individual massage therapist from all liability concerning these injuries stated above that may occur during the massage session and/or contraindications to any herb, product, topical applications during any session

I understand that I or the massage therapist may terminate the session at any time.

It is also understood that any illicit or sexually suggestive, flirtatious remarks, advances, hints, or jokes made by myself (client) at ANY juncture will result in immediate termination of the session and/or client/therapist professional relationship, and I will be liable for payment for the full scheduled appointment.

I have been given a chance to ask questions about the massage therapy session and my questions have been answered.

I understand and agree to the ROAM Bodyworks booking, cancellation, & privacy policies.
